Description The paper aims to study the changes of IITrd ventricle cavity and its floor after neuroendoscopic surgery using the results of MRI planning study and follow up MRI performed after neuroendoscopic surgery. The measurements were completed in 40 patients (21 males, 19 females, average age 44.5 years) after endoscopic third ventriculostomy MRI neuronavigation study (T1W MPR: FOV 300, TR 1870, TE 3.93, slab 1, number of layer per slab 192, slice thickness vrstvy 1.17 mm) Siemens, Magnetom, Symphony 1.5 T, was used for measurements. Statistically significant decrease of CA-CP (the distance between anterior and posterior commissure - third ventricular lenght), IMD (intermamillary distance - third ventricular width) and H (third ventricular height) was shown (Wilcoxon Matched Pairs Test - p < 0.01). No significant changes were proven for parameters describing third ventricular floor - DS-BA (the distance between dorsum sellae and basi-lar bifufurcation) and BA-MB (the distance between basilar bifurcation and mamillary bodies). The study has shown statistically significant changes of third ventricular geometrical parameters, with no changes of parameters describing third ventricular floor, though these changes maybe expected because the surgery takes place at third ventricular floor. The study can be considered to be the starting point of volumetric studies and correlation of clinical changes in hydrocephalic patients with the changes of third ventricular geometry with prognostic sugnificance.
