Description | One potential adjunctive approach to the treatment of stroke patients is the use of neuroprotective and vasoactive drugs in addition to thrombolytic therapy. Methylxanthine derivatives (theophylline and its modified version ethophylline) belong to vasoactive substances. Before the "era" of thrombolytic therapy, agents with a vasodilating effect were a frequently used drug for stroke. The mechanism by which OXANTIL (composite preparation - theophylline, ethophylline) affects the vascular supply of the brain in patients with stroke has been detected, but is missing. The hypothesis of this proposal is that OXANTIL causes vasodilation of cerebral arteries and increases the number of open anastomoses in the cerebral circulation. Vasodilatation facilitates the perfusion and thus the distribution of oxygen (possibly also thrombolytics) to the affected tissue. Not enough massive data are available from the literature, some studies are in the "proof of concept" stage (e.g. https://pubmed.ncbi.nlm.nih.gov/32568651/), but they provide a promising clinical signal that can support future larger clinical trial. We assume that expanding the data on the effect of oxanthil/theophylline/ethophylline from the animal experiment may be beneficial for further study of the use of this substance in clinical research. The lecture presents changes in flow in cerebral vessels detected using transcranial doppler in rats A/ without treatment, or B/ treated i.v. at a dose of 6.4 mg/kg and 1.6 mg/kg ethophylline and theophylline together in one infusion, C/ was i.v. applied separately ethophylline at a dose of 6.4 mg/kg and D/ was administered theophylline at a dose of 1.6 mg/kg. The infusion volume was always 0.5 ml/100 g of body weight. |
