Description Many surveillance studies have shown that heart rate (HR) above 70/min worsens the prognosis of patients with cardiovascular disease. There is a series of drugs that reduce the HR, but not quite enough on one side and on the other side they have many side effects. Recently a specific inhibitor of the If channel -ivabradine has been tested. The authors report on its effect in heart failure on 6558 patients with heart failure of ischemic and nonischemic etiology, NYHA II-IV, with heart rate of 70/min, who had optimal treatment of heart failure, including beta-blockers, in the Shift study. Ivabradine dosed 2 times 5 to 7.5 mg was compared to placebo. The result was a significant reduction in the incidence of the primary goal - a total decrease of hospitalizations for heart failure and cardiovascular mortality of 18% in favor of ivabradine. Also, a decrease of deaths for heart failure was reduced by 26% in the active treatment group. Ivabradine was well tolerated, similar to placebo.
