Description | All patients admitted to the St. Anne's University Hospital (FNUSA) undergo the usual admission screening according to the department's practice: nasal and oral swabs, tracheal aspirate, sputum (according to clinical condition), urine, and wound, if applicable. Newly, from 1 May 2023, in addition to the usual microbiological screening, an admission screening for carbapenemases (CPE) is performed with a rectal swab for patients: with a history of stay in any other health or social care facility in the last year, patients repeatedly treated with antibiotics in the last 3 months, foreigners/migrants from high-risk countries and people hospitalized abroad in the last year. The detection of CPE-producing bacteria in FNUSA is increasing. Pseudomonads predominate in clinical material and carbapenemase production is most frequently detected in urine samples. Escherichia coli predominate in the screening of rectal swabs. Genes encoding CPE are part of mobile genetic elements capable of horizontal transmission and rapid spread (see case reports). We confirm the necessity to implement anti-epidemic measures already at the time of admission of at-risk patients to hospital. |
